be completed prior to our First Appointment
Sigh. I wish there were another way around this paperwork, but I have
not found one yet. I need to get general information about you – age,
address, telephone numbers, etc. on the Referral Form.
If you are asking me to file insurance for you – and
I happily provide this service for all my clients – I will need
for you to complete the Responsible
Party Insurance Information form, and I will need a copy of your
insurance card.
The Notice of Psychologists’ Policies
and Practices... is mandated by law – HIPAA, to be exact – and
that is explained exhaustively in this form.
Finally, I will ask you to read the Psychotherapist-Patient
Services Agreement form so you fully understand the limits of confidentiality
in South Carolina, your rights, information regarding billing, etc. Please,
feel free to ask me any questions about this when you call me or come
in the first time or any time afterward.
- Confidentiality Statement
- The confidentiality clauses in the HIPAA Form and the Therapy Treatment Agreement are governed by Federal and South Carolina law and apply to all psychotherapists. That said, however, I will do my utmost to protect your confidentiality. Only Dr. Walsh has access to all confidential information, and Jan Craven of Integrated Billing Services sees insurance information only. (There are no other staff members in Dr. Walsh's office.)
Insurance & Managed Care
The following managed care and insurance companies are regularly accepted:
CWI Benefits, First
Health, First Mental Health, MHN, Medicare, Mid-South, State Office of
Victim Assistance (SOVA), Tricare/Choice Behavioral Health, and
Value Options.
Other insurances, such as Blue Cross/Blue Shield will reimburse you, not Dr. Walsh. We will gladly file such claims for you, but you will need to pay in full and receive reimbursement from your insurance company.
